Piedmont Medical Center has a wide range of rehabilitative programs in physical therapy. Piedmont Medical Center's team of physical therapists strive to deliver personalized, compassionate care, helping patients of any age regain strength and skills to improve their overall quality of life.

Our Team

Our staff works one-on-one with each patient to help create a personalized plan. Each patient is different: Some conditions may only require a few days of care, while others require several months of therapy.

We offer many treatments in the convenience of 2 (two) centralized locations with certified specialists on-site to help patients on the road to recovery.

Registering for Rehab

Physician offices can fax or email rehab consults directly to the department.

  • For fastest service, referrals should include the specific modality requested (PT/OT/ST) with an order to “Evaluate and Treat” with a specific diagnostic code requiring treatment as well as any recent clinical and demographic information.
  • Orders can be faxed to (803) 981-7450 for Rock Hill or (803) 578-8373 for Fort Mill.
  • Once orders have been received, patients are contacted by our Patient Access Department to be scheduled for a comprehensive 1:1 assessment.
